I found the problem. The Subject Prefix Mod I added was the issue. I went back to an earlier version of my forum and it's fixed.

Yes, I've cleared my cache in each browsers, even used ccleaners afterward. I have a Windows computer and I even used ipconfig to flush out the DNS cache.
